Tools for administering and scoring the “Nine Mental Skills of Successful Athletes’ self-administered questionnaire.

This comprehensive CD package includes a copy of the 30-item questionnaire used to assess an athlete’s mental skills according to the Nine Mental Skills model. The questionnaire can be completed in less than ten minutes and can be hand scored by the athlete or consultant or it may be computer scored using an Excel template.

The kit includes a master copy of the questionnaire, suitable for printing, scoring instructions, the Excel scoring template, reliability and validity data, and permission to reproduce and use the instrument.  Additionally, the CD includes two PowerPoint presentations:  An Introduction to the Nine Mental Skills of Successful Athletes Model and Assessment Using the Nine Mental Skills Questionnaire. All files except for the Excel scoring template are in PDF format.
